What the visits look like
Visits happen in the client's home, at a time arranged directly with them or their family.
Initial assessment
About an hour. The physio talks through goals, checks strength, balance and walking, and looks at falls risks in the home.
Plan and report
A written report and recommendations go to the care manager within 5 business days.
Treatment visits
Usually 45 minutes, building on a home exercise program the client can follow between visits.
Review
Progress is reviewed against the goals, and the plan is adjusted or the program wrapped up.
Funding and cost
We confirm the cost with you before anything is booked. Our quotes include travel.
| Support at Home and Home Care Packages | Funded as allied health through the client's provider. The care manager sends the referral and we invoice the provider using Support at Home service names. |
|---|---|
| NDIS | For plan-managed and self-managed participants, usually from Capacity Building: Improved Daily Living. We are not a registered NDIS provider, so we can't support agency-managed plans. |
| Private | Clients and families can book directly. We'll give you the hourly rate and the likely number of hours up front. |
Questions
Who can make a referral?
Care managers and coordinators, support coordinators, GPs and hospital teams, and clients or their families. For package clients we'll check with the provider before booking.
How soon can a physio visit?
In Sydney and the Illawarra it's often within a week or two. We confirm the earliest available date when we reply to the referral, usually by the next business day.
Are your physiotherapists registered and insured?
Yes. Every physiotherapist is registered with AHPRA, holds $5 million professional indemnity and public liability insurance, has a current police check and works under our agreement.
Can family be involved?
Yes. Family members are welcome at visits, and the physio can show them how to help with the exercise program.
What if the client needs equipment or home modifications?
The physio will recommend it, and our occupational therapists can assess and arrange equipment and modifications through AT-HM funding.
Ready to refer?
Send the service, suburb and how soon it's needed. We'll come back with a date and cost.
