What the visits look like
Visits happen in the client's home, at a time arranged directly with them or their family.
Home assessment
Usually 1 to 1.5 hours. The OT looks at how the client manages at home and what gets in the way.
Report and recommendations
A written report with equipment, modifications and quotes goes to the care manager within 5 business days.
Trial and prescription
Equipment is trialled and prescribed; for modifications the OT writes the scope of works.
Install and review
We coordinate suppliers and builders, then the OT checks the fit and trains the client to use it safely.
Funding and cost
We confirm the cost with you before anything is booked. Our quotes include travel.
| Support at Home and Home Care Packages | Funded through the client's provider. The care manager sends the referral and we invoice the provider using Support at Home service names. |
|---|---|
| NDIS | For plan-managed and self-managed participants. Usually from Capacity Building: Improved Daily Living, with equipment and modifications from Capital supports. We are not a registered NDIS provider, so we can't support agency-managed plans. |
| Private | Clients and families can book directly. We'll give you the rate and the likely number of hours up front. |
Questions
Who can make a referral?
Care managers and coordinators, support coordinators, GPs and hospital teams, and clients or their families. For package clients we check with the provider before booking.
What is AT-HM?
The Support at Home Assistive Technology and Home Modifications scheme funds equipment and modifications separately from the client's ongoing budget. An OT assessment and prescription is usually needed first.
Can you arrange the builder too?
Yes. Our home modifications team builds to the OT's scope of works, so the assessment, quote and install come through one referral.
Are your clinicians registered and insured?
Yes. Every clinician is registered with AHPRA or their professional body, holds $5 million professional indemnity and public liability insurance, has a current police check and works under our agreement.
Ready to refer?
Send the service, suburb and how soon it's needed. We'll come back with a date and cost.
